The following is a list of acceptable documentation, which supports your trading
history, and must be included when filing your Proof of Claim:
- Monthly stock brokerage or investment account statements (reflecting your opening
and closing balances for the months specified on the Proof of Claim Form);
- Trade confirmation slips (reflecting purchase and sale transactions);
- Signed letter from your broker or brokerage firm (on company letterhead) verifying
the transactional information you are providing;
- Deposit or escrow receipt showing your holdings;
- Other satisfactory proof confirming the particulars of each purchase, sale and/or
holding between December 11, 2000 through January 16, 2002, Inclusive.
